If you are requesting CBIT, please list “evaluate and treat for Occupational Therapy CBIT services.” Please be sure to list the referral diagnosis and corresponding ICD-10 code. Referrals can be faxed to (585)824-1301.

$250 per evaluation.

After discussing the treatment plan and establishing specific, attainable goals, each 30–45-minute session will enhance confidence and foster crucial skills.
$50 per session.
Achieve Goals
I am excited to offer guidance and support through coaching and consultation sessions, available in 15-minute intervals.
$25 per 15-minute session.
